Membership Agreement and Liability Waiver
Dear Prospective Member,
Thank you for your interest in joining Rejuvenating, a private membership association dedicated to promoting wellness and rejuvenation through our services. As a condition of membership, we require all members to read, understand, and sign this Membership Agreement and Liability Waiver ("Agreement"). By signing this Agreement, you acknowledge and agree to the terms and conditions outlined below.
1. Assumption of Risk
Rejuvenating offers various services aimed at enhancing personal wellness. You acknowledge that participation in these services and/or presence on Rejuvenating’s premises involves inherent risks, including but not limited to physical injury, dismemberment, or death. You voluntarily assume all risks associated with receiving services or being on the premises, whether known or unknown, and agree to exercise due care and caution during your participation.
2. Release and Waiver of Liability
In consideration of being permitted to join Rejuvenating and utilize its services and facilities, you, on behalf of yourself, your heirs, executors, administrators, and assigns, hereby release, waive, discharge, and hold harmless Rejuvenating, its officers, directors, employees, agents, and affiliates (collectively, "Rejuvenating Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, dismemberment, or death that may be sustained by you while receiving services or while on Rejuvenating’s premises, whether caused by the negligence of the Rejuvenating Parties or otherwise.
3. Grievance and Dispute Resolution
Any disputes, claims, or grievances arising out of or related to your membership, the services provided, or this Agreement shall be resolved exclusively through mediation. You agree to engage in good-faith mediation with a neutral third-party mediator selected by Rejuvenating before pursuing any other legal or equitable remedy. Mediation shall take place in [City, State], unless otherwise agreed by both parties. You expressly waive your right to pursue legal action in a court of law or other judicial forum, except as required to enforce a mediation agreement. The costs of mediation shall be shared equally by the parties, unless otherwise determined by the mediator.
4. Membership Acknowledgment
By signing this Agreement, you confirm that you are of legal age and have the capacity to enter into this legally binding contract. You acknowledge that membership in Rejuvenating is contingent upon signing this Agreement, and failure to comply with its terms may result in termination of your membership.
5. Entire Agreement
This Agreement constitutes the entire understanding between you and Rejuvenating regarding the subject matter herein and supersedes all prior agreements, whether written or oral. This Agreement may only be amended in writing and signed by both parties.
By signing below, I acknowledge that I have read, understood, and agree to be bound by the terms of this Membership Agreement and Liability Waiver. I further acknowledge that I have had the opportunity to seek independent legal advice before signing this Agreement.

Rejuvenating Private Membership Agreement
This Private Membership Agreement ("Agreement") is entered into by and between Rejuvenating, a private membership association ("Rejuvenating"), and the undersigned individual ("Member") on the date specified below.
1. Purpose and Membership Requirement
Rejuvenating requires all clients to become members of the association to access its alternative health services and related offerings. By signing this Agreement, the undersigned acknowledges and accepts membership in Rejuvenating, subject to the terms herein.
2. Background and Legal Protection
Recent actions by state boards, federal regulators, and other entities have sought to restrict access to natural and alternative healthcare options and limit practitioners’ ability to provide such services. These restrictions may stem from interests opposed to non-invasive, non-pharmaceutical approaches to health and wellness. To safeguard members’ access to alternative healthcare, Rejuvenating operates as a private membership association. Membership in Rejuvenating changes an individual’s legal status from a member of the public to a private member, thereby limiting the applicability of certain public regulations that may be misused to restrict alternative medicine practices. This legal framework has been upheld in courts, including the Supreme Court of the United States.
3. Constitutional Protections
Pursuant to the First, Ninth, and Fourteenth Amendments to the Constitution of the United States of America, Rejuvenating and its members are guaranteed the rights to freedom of association, peaceful assembly, and the freedom to contract privately. These rights protect members’ ability to choose alternative healthcare options and ensure freedom from unwarranted interference by state, county, or other authorities. Additionally, these rights safeguard members’ privacy.
4. Member Privacy
Rejuvenating is committed to protecting the confidentiality of all member records. Such records will not be disclosed except upon the written request of the respective member, unless otherwise required by law.
5. Membership Process
To become a member of Rejuvenating, the undersigned must:
- Complete and sign this Agreement;
- Read and acknowledge Rejuvenating’s declaration and policies; and
- Submit the Agreement to Rejuvenating for acceptance.
6. Binding Effect
This Agreement is binding upon the Member and Rejuvenating and shall be interpreted in accordance with the principles of private contract law. By signing below, the Member acknowledges their understanding of and agreement to the terms set forth herein.
